Vital and Enrollment Status in CAREWare tracks a client’s vital and agency enrollment status. The fields are used for demographics tracking and reports, including the Ryan White Services Report (RSR).
Enrollment Status vs. Ryan White Eligibility Records
Enrollment status reflects the client’s original enrollment date in the provider agency. Therefore, the date of enrollment should not change, even if the client leaves the agency and returns at a later date.
Ryan White Eligibility records reflect the periodic review for eligibility. New eligibility dates are added each time a client re-certifies for services.
How to Add Vital and Enrollment Status in CAREWare
Vital and Enrollment Status is located under Demographics in the client record.
1. Click on Demographics in the left-hand menu.
2. Click on Vital and Enrollment Status.
3. Choose the client’s enrollment status from the dropdown list. Options are Active, Inactive/Case Closed, Incarcerated, Referred or Discharged, Relocated, and Removed.
The following Vital and Enrollment Status fields complete the RSR and ADR question:
- Enrollment Date: Q.76 of the RSR New client
- Vital Status: Q.2 of the RSR Client’s vital status at the end of the reporting period
Save: Saves changes to the client’s vital and enrollment status.
Cancel: Returns to the Demographics menu.
4. Complete the field for Enrollment Date.
NOTE: Last Eligibility Status is automatically updated from data in the Eligibility tab.
5. Choose the patient’s vital status from the dropdown list. Options are Alive, Deceased, and Unknown.
6. Complete the Case Closed Date if the Enrollment Status is Inactive/Case Closed, Incarcerated, Referred or Discharged, Relocated, or Removed.
7. Complete the Date of Death field if the client’s Vital Status is Deceased.
8. Click Save.
