ADAP Enrollment History

As a requirement of the AIDS Drug Assistance Program (ADAP), the ADAP Data Report (ADR) is submitted annually. Data elements for a patient’s enrollment are entered in CAREWare within the ADAP Enrollment History of the client’s record.

How to Add ADAP Enrollment History Records in CAREWare

1. Click on Demographics in the left menu.

2. Click on ADAP Enrollment History.

3. Click Add.

 

View: View details of an enrollment record.

Add: Add a new ADAP enrollment record.

Edit: Edit an existing enrollment record.

Delete: Delete an existing enrollment record.

Back: Return to the Demographics menu.

Print or Export: Print or export a list of enrollment records.

Hide/Show Columns: Choose the columns to show in the list of enrollment records.

 

4. Enter the date of the enrollment record.

5. Select the enrollment status from the dropdown list.

6. Click Save.

 

The following ADAP Enrollment Status fields complete the following ADR questions:

Date = Q.14 New Client, Q.15 Date ADAP application was received, Q.17 Date Last Eligibility Confirmation

Enroll Status = Q.18 Client Enrollment Status

Reason for Disenrollment = Q.19 Reason for Disenrollment

How to Add an ADAP Disenrollment Record in CAREWare

1. Click on Demographics in the left menu.

2. Click on ADAP Enrollment History.

3. Click Add.

View: View details of an enrollment record.

Add: Add a new ADAP enrollment record.

Edit: Edit an existing enrollment record.

Delete: Delete an existing enrollment record.

Back: Return to the Demographics menu.

Print or Export: Print or export a list of enrollment records.

Hide/Show Columns: Choose the columns to show in the list of enrollment records.

 

4. Enter the date of the disenrollment record.

5. Select Disenrolled from the dropdown list.

6. Select the reason for disenrollment from the dropdown box.

7. If “Other” is selected for the disenrollment reason, complete the field for Other Disenroll Reason.

8. Click Save.
