Vital Signs

Purpose

Record and maintain a client’s height, weight, pulse, blood pressure, temperature, and BMI.

Who should do this

CAREWare users responsible for entering or updating clinical information in a client record.

Use this guide when you need to

Add a new vital signs record or edit an existing record.

Main warning

BMI is calculated automatically from height and weight and is recalculated when either value changes.

 

Quick path
Find Client > open a client record > Vital Signs > Add or select a record and click View > Edit

What Vital Signs tracks

The Vital Signs tab in client records includes fields for tracking:

  • Height (in.)
  • Weight (lbs.)
  • Pulse (bpm)
  • BP Systolic (mm Hg)
  • BP Diastolic (mm Hg)
  • Temperature (°F)
  • BMI (kg/m²) – automatically calculated based on height and weight

Add vital signs to a client record

Step 1. Click Vital Signs.

Step 2. Click Add.

Vital Signs Menu Options

What it does

View

View an existing vital signs record.

Add

Add a new vital signs record.

Delete

Delete an existing vital signs record.

Help

Access the Knowledge Base article for vital signs.

Print or Export

Print or export the list of vital signs records.

Column Visibility

Hide or show columns in the list of vital signs records.

Step 3. Complete the fields for vital signs.

Step 4. Click Save.

Note: If height and/or weight are edited, BMI is automatically recalculated based on the new values.

 

How to confirm it worked

  • The expected client record shows in the list of vital signs records.
  • BMI auto-calculates when height and weight are entered

Why does BMI not auto-calculate?

Verify that height and weight are entered.

Does the record need to include all the vital sign values?

Users can add one or more vital signs in a record.
